require daily assistance with at least one Activity of Daily Living (ADL) i.e, bathing, dressing, eating, continence care, ambulation or transfer

MassHealth Itss must approve your clinical eligibility



be eligible for MassHealth (standard, SCO, or be able to pay privately)



have a medical diagnosis and need daily personal care



be able to communicate his/her needs (ok to be confused or forgetful)



not be harmful to self or others



be compliant with plan of care
